New Cigna Partnership Aims to Drive Senior Post-Acute Care Outcomes

With sights set on improving patient outcomes and reducing costs, a partnership between Cigna-HealthSpring and naviHealth aims to help hospitalized seniors navigate post-acute care options upon discharge. 

Under the program, naviHealth’s team of nurses and licensed therapists will engage and assess Cigna-Healthspring members in the hospital to educate them and their caregivers on their options for post-acute care and rehabilitation. 

Upon discharge, naviHealth will work with patients and their care teams in skilled nursing facilities (SNFs) to continually monitor and update patients’ care plans, using its technology and field-based care managers to foster care coordination.

“This new collaboration will help our members more efficiently navigate their options once leaving the hospital, and further our goal of continually improving the care delivery and patient experience across our markets,” said Patrick Foley, president of Cigna-HealthSpring’s Mid-Atlantic and Pennsylvania market.

As a result of the newly formed partnership, Cigna-HealthSpring expects not only to see improvements in clinical outcomes for its members, but also enhanced satisfaction resulting from greater collaboration between care managers and post-acute providers. 

The combination of naviHealth’s decision support technology and analytics coupled with Cigna’s 100,000 membership base in the Mid-Atlantic and Pennsylvania markets has the company confident in producing greater health outcomes at lower costs.
